Full Course Title: Supervision and Operating a Community Health Worker program within a Chronic Disease Medical Home

This course is designed to assist any program who wants to learn to set up a team working with patients with a chronic disease. In addition, it also supports the care teams and discusses burn out and how to address any concerns related to their own safety within this program. There are four (4) lessons in this course: the medical care team in sickle cell disease (SCD) care; supervision and daily operations of a CHW program; burnout, compassion fatigue, work related stress injury and psychological safety; and documentation, data collection and evaluation.

Course Outcome:
Describe how a medical home for patients with SCD is developed describing the critical components including staffing, financing, daily operations, supervision, promoting psychological safety and monitoring.
