Description

People living with a chronic disease, such as sickle cell disease, face daily challenges. Many of these challenges are related to where they live, their housing, income, education, and access to food. These are called social determinants of health. There are many ways to address these challenges, from helping one person at a time to making changes in communities and policies. One proven way to improve health and support patients is by including a community health worker (CHW) on the care team. CHWs help patients manage their health and connect them to important resources. Adding a CHW to a medical team can lead to better health outcomes. There are five (5) lessons in this course: roles and responsibilities; establishing relationships; care coordination and case management; conducting a home visit; and confidentiality in providing CHW services.

Course Outcome:
Identify the role and responsibilities of community health workers (CHW) working with patients with chronic medical needs such as in SCD care.
